New Client Intake Form
The first step to booking an appointment at ACE is filling out a brief New Client Intake Form. The New Client Intake Form consists of general contact information and information on the prospective student which includes: previous and current therapy, communicative skills, repetitive and self-stimulatory behaviors, and your child’s strengths and abilities and general liability claims. This information allows our provider to plan accordingly and prepare for lessons. ACE accepts all student regardless of abilities, age, previous exposure, stims or level of aggression.

After completing the Client Intake Form, please email it to ACE (firstname.lastname@example.org) and, if possible, list which services you are currently interested in.
There are separate registration forms for 2-a-day (Thursday/Friday) sessions and camps. Each registration form lists available dates and time slots. When filling out the form, ACE requires you to indicate a first and second choice for dates to assist in the scheduling process. Please note- time slots will be assigned based on availability. These services require a non-refundable, non-transferable half deposit in order to secure your session(s). Instructions for submitting your half deposit will be included in your confirmation email.
